The active ingredient of Tirzalgid™ is tirzepatide — the first-in-class dual agonist of glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. By activating both incretin pathways at once, it enhances insulin secretion, suppresses glucagon release and slows gastric emptying.
Tirzepatide reduces total body weight and adipose tissue mass by lowering energy intake. The dual GIP/GLP-1 mechanism provides a stronger decrease in appetite and food cravings than single GLP-1 agonists, improves control over food intake and shifts preference away from high-fat foods. In clinical trials tirzepatide achieved average weight loss of up to 20–22% of baseline body weight.Tirzalgid™ lowers blood glucose levels in adults with type 2 diabetes, reducing both fasting plasma glucose and glycated hemoglobin (HbA1c) more effectively than selective GLP-1 agonists.

Tirzalgid™ is not insulin — it belongs to a different class of drugs. Moreover, its mechanism of action is aimed at reducing insulin resistance, i.e. normalizing the sensitivity of body cells to the patient's own endogenous insulin, while tirzepatide also improves the function of pancreatic beta-cells. In other words, the drug restores the body's natural regulation of glucose, which allows type 2 diabetes to be brought under control, improves disease prognosis and can significantly delay the need for insulin replacement therapy.
For type 2 diabetes mellitus Tirzalgid™ is indicated for long-term use, because it reduces insulin resistance and has a beneficial effect not only on the pancreatic cells that produce insulin, but also helps normalize systolic blood pressure, improve the lipid profile and much more. When Tirzalgid™ is used for weight loss, the duration of the course depends on many factors: how long a person has been living with increased body weight, the volume and caloric content of their daily diet before starting therapy, the rate of weight loss, physical activity, sleep duration and more. The optimal weight loss for the body is on average 5% over 3 months. Firstly, more intensive loss is stressful for the body, and secondly, with rapid weight loss a factor known as "obesogenic memory" is activated — the regain of body weight after its successful reduction.
